2. ELIGIBILITY
2.1 The candidate holding BAMS or equivalent degree recognized by BHU and CCIM.
2.2 The candidate holding MBBS degree recognized by MCI.
2.3 The candidate should have passed I.Sc. (Biology) or equivalent examination recognized by BHU.
2.4 All the candidates (except MBBS) must have qualified with Sanskrit as one of the subjects in Intermediate or equivalent examination or as a part of BAMS examination recognized by Banaras Hindu University.
2.5 The candidate must have completed minimum 12 months Internship (4 ½ years BAMS Course) / 6 months Internship (5 years BAMS Course) by 31.7.2012 after passing the final professional examination.
2.6 The maximum age limit for the General and OBC candidates will be 35 years as on 31.7.2012 (relaxed up to 5 years in case of SC/ST candidates). There is no maximum age limit for the sponsored candidate.
2.7 In case of the foreign nationals the degree must have approval of the CCIM or MCI as equivalent to the corresponding examination for admission.
2.8 Temporary registration as medical practitioner of foreign nationals is essential. CCIM or MCI should approve their registration valid for study period.
2.9 Institutional Candidate means who have passed final professional BAMS examination of December 2010 from Faculty of Ayurveda, IMS, BHU and likely to complete their internship on or before 31-7-2012.
3. AVAILABILITY OF APPLICATION FORMS
3.1 The application form can be downloaded from websites www.bhu.ac.in or www.imsbhu.nic.in.
3.2 It can also be obtained by post by sending a written request to the Director, Institute of Medical Sciences, BHU, Varanasi -221005 through the registered post along with a MICR Demand Draft/ Bankers cheque of Rs 100/- drawn in favour of Director, Institute of Medical Sciences, payable at Varanasi along with two self addressed slips (06 X 10cm). The last date for receiving requisition for supplying application form by post is 3rd April, 2012. Money orders or cheques will not be
accepted.
3.3 It can also be obtained from counter of office of the Director, Institute of Medical Sciences, BHU, Varanasi on submission of a MICR Demand Draft/ Bankers cheque of Rs 100/- in favour of the Director, Institute of Medical Sciences, payable at
Varanasi, between 16th February, 2012 and 13th April, 2012.
4. SUBMISSION OF APPLICATION FORMS
4.1 Candidate must fill all the four pages of the application form.
4.2 The application form, completed in all respects should reach the office of the Director, Institute of Medical Sciences, Banaras Hindu University, Varanasi – 221005 on or before 16.04.2012 (5.00 PM). The envelope containing the
application should be superscribed “APPLICATION FOR Ayurveda Vachaspati [MD (Ay)]/Ayurveda Dhanwantari [MS (Ay)] COURSE - 2012”. Advance photocopy, Fax or e-Mail copies of the application will not be accepted.
4.3 Duly completed application must be accompanied with an MICR Demand Draft of Rs 1,600/- in case of general and OBC category (Rs 1000/- for SC and ST) (applicable to both internal and external candidates) drawn in favour of DIRECTOR,
IMS, BHU on a nationalized bank (preferably State Bank of India/ Bank of Baroda) and payable at Varanasi. Money orders or cheques will not be accepted.
4.4 Incomplete applications in any respect will be summarily rejected and no correspondence in this regard will be entertained.
Applications received after the last date will not be entertained on any ground including postal delay.
4.5 All passport size photographs must be identical, snapped with front profile, showing full head, face, shoulder and both ears, and taken without wearing cap, hat or Sunglasses. Polaroid photographs are not accepted. Photographs must be taken on or after 3rd February, 2012 with a placard indicating the date and name of the candidate. The photograph in A on Page 1 should be attested by a Gazetted officer and photographs in B and C (on page 3 and 4) are to be self signed. Signature of the attesting officer/candidate should begin on application form and then extend over the photograph. The thumb impression on page 4 of the application also to be attested by Head of the Institution/Gazetted Officer.
4.6 The candidate who is in service (Government/Semi Government/Statutory body) must apply through proper channel with endorsement certificate of the employer on the proforma provided in the application form.
4.7 Provisional admit card shall be sent by the Registered post to the candidate. If an eligible candidate does not receive the admit card, he/she may contact the Office of the Director, Institute of Medical Sciences, B.H.U., Varanasi on Tuesday, 12th June 2012 between 10.00 AM to 5.00 PM along with 2 copies of passport size photographs identical to those pasted in original application form. No duplicate admit card will be issued on the day of examination i.e., Wednesday, 13th June 2012.

1. ELIGIBILITY:
D.M. : Medical graduates holding the degree of M.D. (Medicine) or M.D. (Paediatrics) or DNB in respective subjects or equivalent degree recognized by the Medical Council of India.
For the DM (Endocrinology) MD/DNB in Biochemistry candidates are also eligible.
M.Ch. : M.S./DNB (Surgery) or equivalent degree recognized by the Medical Council of India.
For the Surgical Oncology, the candidates having MS/DNB in ENT, Orthopedics and
Obst. & Gynaecology are also eligible.
For both the above courses the candidates must have passed the MD/MS/DNB Examination
before 31st May 2012.
2. DURATION AND RECOGNITION OF THE COURSE:
a) Duration of DM/MCh course is 3 years
b) The degrees of M.Ch. (Paediatric Surgery, Plastic Surgery, Neurosurgery, Urology) and DM (Endocrinology, Gastroenterology, Nephrology) are recognized by the Medical Council of India. The recognition of other degrees is under process.

Sponsored Category:
i) For sponsored category seats, only those candidates who are in permanent service of any Government or statutory body are eligible.
ii) They must apply with a sponsorship certificate on the format given in the application form (duly signed by the Employer with date, designation and seal) that he/she is being sponsored for undergoing the course applied for
and that he/she will get leave for the full duration of three years of the course of study, with full pay or half pay, and the assurance that after completion of the course he/she will be taken back in service by the employer concerned.
iii) The selection of the candidates will be on the index (out of 100) calculated on the basis of their aggregate marks in the MBBS, and PG examination (number of attempts) and merit in interview.
Sponsored candidates will not have to appear in the Entrance examination.
5. AVAILABILITY OF APPLICATION FORMS:
a) It can be downloaded from BHU Web. Site www.bhu.ac.in . & www.imsbhu.ac.in
b) It can also be obtained from the office of the Director, Institute of Medical sciences, BHU, Varanasi on submission of a MICR D/D of Rs. 100/- in favour of the “Director, Institute of Medical Sciences, BHU”, payable at Varanasi along with self addressed envelop (25 x 16 cm) by post till 30th April, 2012 and in person till 1st May 2012.
6. SUBMISSION OF APPLICATION FORMS:
i) The eligible candidates may apply by filling up all the six pages of application form enclosed herewith in his/her own clean handwriting and all the entries in the form should be filled by the same writing material.
ii) A candidate can apply and appear for one subject only for the regular residency seat. The same is applicable for the sponsored candidates as well.
iii) The application must be accompanied with attested copies of all the mark sheets / certificates / degrees / registration number certificate and other documents (from High School to MD/MS/DNB)
iv) The completed application form in all respects along with all necessary enclosures should reach the office of Director, IMS – BHU on or before Monday, 7th May 2012, 5.00 PM. The envelope containing the application should be superscribed "APPLICATION FOR D.M./M.Ch. COURSE AUGUST 2012". Applications sent by
Fax, E-Mail, Photocopies or any other form will not be entertained. Only the original Application form and form downloaded through BHU website will be accepted.
v) Duly completed application must be accompanied by a MICR Demand Draft drawn on a nationalized bank and payable at Varanasi in favour of DIRECTOR, IMS, BHU of Rs. 2000/- (Rupees Two Thousand only).
Demand Draft submitted with this form includes examination fees and is not refundable in any case.
vi) Candidates who are in service of Government/Semi-Government/Statutory Body must apply through proper channel with endorsement certificate of the Employer on the proforma as provided in the application form.
vii) Incomplete applications in any respect will be summarily rejected and no correspondence in this regard will be entertained. Applications received after the last date will not be entertained on any ground including postal delay.
viii) All passport size photographs must be self attested, identical, snapped with front profile, showing full head, face, shoulder and with both ears and taken without wearing any Cap/Hat/Sun glasses. ix) Provisional Admit Card shall be sent by Registered Post to the candidates. If an eligible candidate does not receive the admit card, he/she may contact the office of the Director, Institute of Medical Sciences, BHU, Varanasi on 30th June 2012 between 10.00 AM to 5.00 PM along with 2 copies of self attested passport size photographs identical to those pasted in Original application form.
Duplicate admit card will NOT be issued on the day of the examination. i.e. 01st
July 2012.
7. EVALUATION:
i) The test paper will consist of 100 objective type questions and the Examination will be of 100 minutes duration.
ii) Four marks will be awarded for each correct answer. There will be negative marking for incorrect answer, and One mark will be deducted for every incorrect answer.
iii) The qualifying marks are 50 percent of the aggregate.
iv) No scrutiny/revaluation of the answer sheet of the Admission test shall be allowed on any ground.
8. SELECTION:
Subject wise merit list will be declared & posted on the BHU web site www.bhu.ac.in
i) The candidates will be selected for registration/ admission strictly based on merit in the written test. In case of equal marks in the test the inter-se-ranking of the candidates shall be determined in the following order:-
(a) In case of candidates having equal marks in the test, the merit shall be decided on the basis of number of failures at the MD/MS/DNB examination. One mark will be deducted, out of the total marks obtained by the candidates in the written test, for each failure.
(b) In case of candidates having equal marks vice (a) above, the merit shall be decided on the basis of percentage of aggregate or marks obtained by the candidates in MBBS (Final Examination).
(c) In case of candidates having equal percentage of aggregate marks at the MBBS (Final) Examination vice (b) above, the merit shall be decided on the basis of percentage of marks obtained by the candidates, in subject of Medicine (for selection to D.M. course) and Surgery (for selection to M.Ch.course), in the MBBS examination.
(d) If the marks at the above vice (c) happen to be the same, the date of birth would be the basis i.e. the candidate senior in the age would rank higher.
9. GENERAL:
i) The sponsored/deputed applicants will be treated as supernumerary candidates and in case of their selection and registration/admission and joining for the course, they will not get any payment from the Institute/University.
ii) No private practice of any kind shall be permitted during the entire duration of the DM / MCh course.
iii) The decision of the Director, Institute of Medical Sciences, B.H.U., shall be final and binding on the candidates in respect of the Admission test.
iv) The selected candidates will have to appear before the admission committee for counseling and also required to sign a bond on non-judicial stamp paper of Rs. 100/- (to be purchased from Varanasi or any district of State of Uttar Pradesh) on the format to be supplied by the Directorate before he/she is permitted to join the DM / MCh course.
v) The admission and continuance of Registration/Residency of the candidate shall be subject to the BHU Act, Statutes, Ordinance, Rules and Regulations and orders as may be applicable from time to time and the aforesaid bond. The registration/ admission may be cancelled without any notice if the supervisor of the Department is not satisfied with the work and/or conduct of the candidate.
vi) The decision of the Postgraduate Medical Board / Faculty of Medicine, Institute of Medical Sciences - BHU in all the matters of admission/registration shall be final and binding on the candidate.
vii) Mere submission of application and appearing and qualifying in the Admission test will not entitle the candidates to claim admission in any of the above course. The candidates must note that if it is found that he/she is not eligible for the Admission test, the Directorate will not issue the Admit Card and will not take any responsibility in this regard.
viii) No TA/DA will be admissible for appearing in the Admission test, and for joining the course.
ix) Candidate will have to submit documentary proof at the time of admission / registration that their medical degrees are recognized by MCI.

Applications are invited for the course of B.Sc Nursing in the College of Nursing at Institute of Medical Sciences, B.H.U., Varanasi & B. Pharm (Ay.) at The Rajiv Gandhi South Campus, Barkacha, Mirzapur Dist., under Institute of Medical Sciences, BHU, Varanasi.
Eligibility: Intermediate 10+2 OR equivalent from the recognizedboard/university with Science (Physics, Chemistry, Biology) & English with minimum 50% aggregate marks (PCBE) for General/OBC category and 40% for SC/ST category.
The age of the candidates should be between 17 to 25 years for all categories as on 31-07-2012.

How To Apply
The sale of application form and information leaflets will start from 19.03.2012. The same can be purchased in person from Institute’s counter or by post after submitting a MICR Demand Draft of Rs. 100/- (Rupees One Hundred) along with a self addressed envelope of 25x 16 cm and a request letter addressed to The Director, IMS, BHU, Varanasi – 221 005 mentioning the appropriate course and full address with contact telephone number. The envelope must be superscribed as “Request for application form for DM/MCh August 2012.” The same can also be downloaded from BHU website www.bhu.ac.in & www.imsbhu.nic.in
The completed Application Form with all enclosures must reach the office of the Director, IMS, and BHU on or before 07.05.12. The MICR D/D drawn in favour of “Director, IMS, BHU” payable at Varanasi of Rs.2000/- for DM/MCh Course (Residency/Sponsored) must accompany the Application form.

The 'Application Form' alongwith 'Information Bulletin' (separate for each Course) can be purchased from the Office of Controller of Examinations, BHU, Varanasi or some other designated offices of the Banaras Hindu University on all working days from 11.00 AM to 3.00 PM by an amount of Rs.50/- (Rupees Fifty only), in cash.
ENTRANCE FEE:
For admission to a course, a candidate will be required to submit a filled-in 'Application Form' for the purpose.
(Note: The application form for admission can also be downloaded from : www.bhu.ac.in ).
Candidates should pay the Entrance Fee along with the Application Form for the amount mentioned below:
All courses : Rs. 700 (Others) & Rs. 300 (SC/ST)
The Entrance Fee is required to be deposited by a "Challan" in any branch of State Bank of India (CBS Branches). The Challan will be in three-folds which is attached in the Information Bulletin at the last page. The candidate will be required to visit any branch of the State Bank of India of his/her choice where he/she will be required to deposit the requisite amount of money to the Bank and the Bank will retain one fold of the Challan and remaining two folds of the Challan mentioning the journal number, date of deposit of money will be given to the candidate of which one will be retained by the candidate for his record and the other will be required to be submitted alongwith the Application Form, in the envelope provided for Application Form, to the Office of the Controller of Examinations, Banaras Hindu University either by hand or through Registered Post / Speed Post / Courier within the stipulated date mentioned in the Information Bulletin.

The Banaras Hindu University will hold All India COMPETITIVE WRITTEN TEST for admission to MBBS course (84 seats) in TWO stages: PMT-2012 SCREENING and PMT-2012 MAIN, and for BDS (38 seats), BAMS* (50 seats) and BPharm (33 seats) courses in ONE stage: PMT-2012 Screening.
PMT-2012 SCREENING shall be held on Wednesday, the 16th May 2012 at Varanasi, Delhi, Kolkata, and Hyderabad Centres to screen candidates for eligibility to appear in PMT-2012 MAIN examination for MBBS course which will be held on Sunday, the 17th June 2012 at Varanasi Centre only. For BDS / BAMS* / BPharm courses, candidates will be selected on the basis of their merit in the PMT-2012 SCREENING only.
ELIGIBILITY:
Qualifications - Passed Intermediate / 10+2 or equivalent examination in English, Physics, Chemistry and Biology (Zoology + Botany) taken together;
(i) For MBBS / BDS / BAMS – a minimum of 50% marks for General and 45% for General- PC candidates and 40% marks for OBC# / SC / ST and respective PC candidates;
(ii) For BPharm – a minimum of 60% marks for General / OBC and respective PC candidates and 55% marks for SC / ST and respective PC candidates. Candidates appearing in qualifying examination may also apply
AGE: Completed 17 years but not crossed 25 years as on 31st December, 2012 (Candidates born after 1st January, 1996 and on or before 31st December, 1987 need not apply).

Applications have been invited by Banaras Hindu University, Varanasi for admission to BSc Nursing and BPharm (Ayurveda) for the Academic Year 2012 in the College of Nursing at Institute of Medical Sciences, B.H.U., Varanasi & The Rajiv Gandhi South Campus, Barkacha, Mirzapur Dist., respectively.
ELIGIBILITY:
Intermediate 10+2 OR equivalent from the recognized board/university with Science (Physics, Chemistry, Biology) & English with minimum 50% aggregate marks (PCBE) for General/OBC category and 40% for SC/ST category.
The age of the candidates should be between 17 to 25 years for all categories as on 31-07-2012.

(b) By hand from the Office of the Director, Institute of Medical Sciences, BHU, Varanasi by paying Rs. 50/- in Cash – from 14-1-2012 to 22-03-2012.
(c) By Post: From the Office of the Director, Institute of Medical Sciences, Banaras Hindu University, Varanasi-221005 by sending a written request through Registered Post along with Crossed Demand Draft/Bankers Cheque (MICR only) of Rs. 100/- of a Nationalized Bank, drawn in favour of “The Director, Institute of Medical Sciences, BHU” payable at Varanasi.
Application form duly completed must be sent through Registered Post so as to reach THE DIRECTOR, INSTITUTE OF MEDICAL SCIENCES, BANARAS HINDU UNIVERSITY, VARANASI-221 005 latest by 28-3-2012. A Demand Draft Rs. 1000/- for General/OBC Category and Rs. 600/- for SC/ST Category in favour of the Director, IMS, BHU payable at Varanasi has to be attached with the completed form.
Commencement of Entrance test: 03-06-2012 (Sunday) from 10.00 am to 12.00 noon at Varanasi

Eligibility
QUALIFICATIONS - Passed Intermediate / 10+2 or equivalent examination in English, Physics, Chemistry and Biology (Zoology + Botany) taken together;
(i) For MBBS/ BDS/ BAMS – a minimum of 50% marks for General and 45% for General-PC candidates and 40% marks for OBC#/SC/ST and respective PC candidates;
(ii) For B.Pharm. – a minimum of 60% marks for General / OBC and respective PC candidates and 55% marks for SC/ST and respective PC candidates. Candidates appearing in qualifying examination may also apply.
AGE: Completed 17 years but not crossed 25 years as on 31st December, 2012 (Candidates born after 1st January, 1996 and on or before 31st December, 1987 need not apply).

(b) BY POST : From the Office of the Director (PMT-Cell), Institute of Medical Sciences, Banaras Hindu University, Varanasi-221 005 by sending a written request through Registered Post along with Crossed Demand Draft/ Bankers Cheque (MICR only) of Rs. 1600/- for General/OBC candidates and Rs. 1100 for SC/ST candidates of a Nationalized Bank, drawn in favour of “The Director, Institute of Medical Sciences, BHU” payable at Varanasi.
Application form will be sent by Speed / Registered Post. However, Institute will not be responsible for any loss or postal delay.
The request for application form must include the name and address of the candidate, and full particulars of the enclosed DD/BC (write candidate’s name and address on the back of DD). Two self-addressed, preferably typed, white paper slips of 10 cm X 6 cm size must also accompany it.
The requisition for PMT Application Form should be super scribed “Request for Application Form for PMT-2012, BHU”. Last date of requisition for PMT form by post is 27.02.2012.
Application Forms from the Office of the Director (PMT-Cell), Institute of Medical Sciences, BHU, Varanasi-221005 can be obtained by POST only and not in person upto 27.02.2012.
