I. Applications from eligible candidates are invited for admission to the Post Graduate Super Speciality Course in Cardiology(DM Cardiology), in the Academy of Medical Sciences, Pariyaram, Kannur for academic session starting September 2011.

II. Duration of the Course: Duration of the Course shall be three years from the date of commencement of the course. All candidates should complete 3 years from their date of joining. The date of commencement of the course will be 29/09/11.

III. Eligibility for admission
a) Nativity: Applicant should have satisfied the following conditions
i) Should be Indian citizen
ii) Have passed the PG Degree (MD in General Medicine/Paediatrics/Respiratory Medicine) from any of the Medical colleges, in Kerala/ any of the states in India
iii) Indian citizen having resident status in any of the Indian States/ Union territories.
b) Basic Academic Qualifications:
(i) MBBS Degree
(ii) Post Graduate Degree (MD in General Medicine/Paediatrics/Respiratory Medicine) from the Universities in Kerala or any other University recognized by any of the Universities in Kerala as equivalent thereto OR appropriate Diploma of National Board (Dip. N.B) of examinations, New Delhi
(iii) Registration: Candidates should have got their MBBS and PG Degree registered permanently with their State Medical Council and it should be submitted at the time of counseling c) Age: Upper age limit for merit candidates will be 45 years and 52
for candidates of Medical Education Service.


IV All candidates, shall have to qualify the respective super speciality Entrance Examination conducted by the Commissioner for Entrance Examinations, Kerala or respective state entrance examination or All India Entrance Examination.

V Fees: [to be fixed by Management]

VI How and when to apply:
Application available at office of Principal ACME on all working days from 10 Am to 5 Pm on payment of ` 3,000/- from 28/08/11. Online service provided.

Application available on surface mail on payment of application fees.

Last date of receipt of filled up application on the office of principal will be 23/09/11.

Online application should carry draft for `3000/- payable to Principal, Academy of Medical Sciences as application fee.

VII Certificates/ Documents to be attached along with the application form Attested Xerox copies of the following documents:
(i) M.B.B.S Degree Certificate
(ii) Post Graduate Degree/Dip. N.B. Certificate
(iii) Certificate to prove age
(iv) Relevant page of the Secondary School Leaving Certificate/ Indian Passport/ Birth Certificate showing nativity/ citizenship.
(v) In respect of candidates claiming nativity under clause III(a)(III) a certificate, issued by the Director of Medical Education of that state, has to be submitted in the prescribed format to the effect that “admission to PG super
Speciality Courses in Government colleges of that state is open to all Indian citizens irrespective of the domicile”
(vi) Any other certificate required along with the application
(vii) Fee receipt of the amount (application cost) paid in original Certificates in the application form:
(viii) In the case of non- Keralite candidates, nativity Certificate as per Clause III (a) (ii), should be obtained in the application form, from the Home department/ revenue authorities of the concerned state to show that that he/she is a resident of that state and is a citizen of India.

Additional Certificate(s) to be submitted along with the application
(a) ‘No Objection Certificate’ from the employer if employed.

VIII SELECTION:
Candidates seeking admission for Super Speciality Course(DM Cardiology) will be selected on the basis of their merit at Entrance Examination, (Kerala 2011/All India 2011/ other state entrance examination) and willingness to pay the fees to be fixed by management.

IX. Liquidated damages & Execution of Bond:
The applicant selected for the Super Speciality Course has to execute two bonds as shown below in stamped paper (all pages of the bond should be in stamped paper) of the total value of `200/- (Kerala stamp paper) each, at the time of joining the course to the effect that:
“ He /She shall not discontinue the Super Speciality Course he/she has been allotted and if discontinues the Super Speciality Course he/she shall pay 70 lakhs (Rupees Seventy lakhs only)”

X. General
Notwithstanding anything contained in the Prospectus, Institution at any time of their own accord or otherwise, revise or amend the prospectus.
