Application form and Prospectus
a. The prescribed Application Form along with the Prospectus can be obtained in person from the following places on payment of Rs.360/- (including service charges of Rs.10/-) till the last date prescribed for the sale of Application Forms: Andhra Bank: S.K. University Campus Branch, Anantapur.
b. The Application Form and the Prospectus can be downloaded from the website:www.sku-dde.com But such an application should be accompanied by the counterfoil of on-line transfer or a Demand Draft for Rs.350/-.
The filled in Application Form should be sent by Registered/Speed Post to Director, Directorate of Distance Education, Sri Krishnadevaraya University, Anantapur – 515 055, Andhra Pradesh.
The following should be sent along with the Application Form:a. The counterfoil of the online transfer or the Demand Draft towards the Course Fee. b. Original Degree/Provisional Pass Certificate and Marks Statements along with 2 sets of Xerox copies.c. Xerox of SSC or its equivalent.d. One stamp size photograph to paste on the Identity card.e. Three passport size photographs
