Those candidates with Debit Cards and Net banking with the currently displayed banks may proceed with payment of Application fee. Candidates intending to use Credit Cards and Debit cards/Net Banking from other banks are requested to wait till 7th April for completion of Payment.

For candidates who have completed 10th Std. and 12th Std. from Indian Boards outside India, you may use the option ‘Outside India’ in the column ‘State of Passing’ for 10th Std. and 12th Std. This option will be available in the relevant drop down box shortly.

Applications have been invited by The Registrar, Christian Medical College, Ludhiana-141 008, Punjab for admission to General Nursing & Midwifery course of year 2014.

REQUIREMENTS
Age:
Applicants for admission must be at least 17 years of age on 31st December 2014 and not more than 35 years of age.
Applicants born after 31st December 1997 or before 1st January 1979 are not eligible to apply for admission to General Nursing &
Midwifery course.
The date of Birth being shown on the Matriculation certificate/Higher Secondary certificate/equivalent is acceptable.

ACADEMIC
In order to be eligible to apply for admission to the first year General Nursing & Midwifery course, an applicant must have
passed 10+2 class or its equivalent with Science (Physics, Chemistry & Biology) or arts with aggregate of 40% marks.

CHRISTIAN APPLICANTS :
1) All Christian applicants must provide Photostat copies of the following certificates along with the application:
a) Baptism Certificate
b) Confirmation Certificate (if applicable)
c) Church Membership, with date and denomination of the Church with address
2) All candidates applying under Christian categories (1 to 4) have to pass a Bible test also. Any applicant under Christian Category who does not pass (obtain 40% marks) in the Bible test will be considered in the APPROPRIATE open category.

HOW TO APPLY
Prospectus and Application Form etc. for General Nursing & Midwifery program for September 2014 may be obtained at the counter of the main hospital reception the C.M.C. Hospital on cash payment of Rs. 500/- (Rupees Five Hundred only) till 20th August 2014 and also can be downloaded from the link: http://cmcludhiana.in/registrar-office/gnmappform.pdf
Draft should be made payable to ?Christian Medical College Ludhiana Society? drawn on any scheduled bank, payable at Ludhiana.
Applicant should send the draft to the Registrar, Christian Medical College, Ludhiana-141 008, Punjab along with a covering letter stating the complete address of the applicant including pin code and mobile / phone number.
Last date of receipt of application along with enclosures in the Registrar’s Office is 22nd August 2014 by 5.00 p.m.

Undergraduate courses-2014
Admission to MBBS, BDS, B.Sc. Nursing and BPT (collectively called as UG courses) of this institution,
are on the basis of merit obtained in the Admission Written Test (UG – AWT - 2014) conducted by Christian Medical College Ludhiana Society. All the admissions are made with the approval of Baba Farid University of Health Sciences, Punjab; the Government as may be applicable; Medical Council of India (as may be applicable); Dental Council of India (as may be applicable); Punjab Nurses Registration Council and Indian Nursing Council (as may be applicable) and selection guidelines of the Christian Medical College, Ludhiana.
These are subject to alterations, amendments and modifications as may be considered necessary from time to time by the authorities concerned. Candidates, when applying, are requested to specify the category they belong to, as given on page 11 & page 12.
The primary aim of the Christian Medical College Ludhiana is to make a significant contribution to the health standards of all communities of our nation with a special emphasis on health care in the rural underserved and unreached areas/communities in India. This mandate is fulfilled by our
graduates who serve in such areas after their training.

Open Category
Any candidate irrespective of religion, caste, or domicile and who is an Indian National, can apply under this category. They will not have a service agreement but are encouraged to serve in the rural mission hospitals to understand the basic needs of the country.
Christians with Service Agreement- MBBS & Christian Minority Categories -BDS, BSc.N, BPT The Christian candidates applying in this category should indicate in the Application form and will have to take a Bible Test. Those candidates who desire to serve in a Mission Hospital of the Christian Medical
College Ludhiana Society, following MBBS, are required to submit a Letter of Service Committment by one of the member bodies of the Christian Medical College Ludhiana Society. All MBBS candidates selected through UG AWT 2014 in this category will have to enter into a service agreement without which admission will not be granted in this category. Service Agreement for MBBS candidates is for a minimum period of two years after completion of the course in a hospital or institution managed by Christian Medical College Ludhiana Society or its member Body/Church. Christian candidates applying in MBBS who do not wish to commit to serve in a Mission Hospital should apply in the Open category.
In keeping with the mandate of the Christian Medical College Ludhiana Society to reach out to all
states with emphasis on the rural underserved and unreached areas / communities of our nation, to provide training facilities to candidates who will dedicate themselves to the healthcare of these states they represent, which would otherwise have been deprived, seats in MBBS and BDS have been earmarked area-wise from States and Union territories (Pg. 11). Candidates applying to these categories should clearly indicate this in the application form by correctly filling the domicile field.
Admission will be entirely on merit, based on the Admission Written Test.


ELIGIBILITY CRITERIA FOR UG AWT 2014
AGE REQUIREMENTS
Applicants must be at least 17 years of age on 31st December 2014 (the date of birth being shown on the Matriculation or Higher Secondary Certificate or equivalent certificate). Applicants born after 31st December 1997 are not eligible to apply for admission to Undergraduate courses starting in July 2014.

NATIONALITY
Applicants must be citizens of India for UG AWT 2014. Persons of Indian Origin with Citizenship of other countries may apply under NRI category for which a separate prospectus is available. No other foreign
national will be admitted.

ACADEMIC REQUIREMENTS
A candidate for admission to MBBS and BDS must have passed in the subjects Physics, Chemistry Biology and English individually and must have obtained not less than 50% marks in English and a minimum of 50% marks (40% for SC/BC) in Physics, Chemistry and Biology taken together at the qualifying examination (as per MCI Regulations 1997).
For admission to B.Sc. Nursing and BPT course a Candidate must have passed in the subjects of Physics, Chemistry, Biology and English individually and must have obtained a minimum of 45% marks taken
together in Physics, Chemistry , Biology and English in 10+2 examination.

General school information
He / She has passed qualifying examination as under: -
1. The higher secondary examination or the Indian School Certificate Examination which is equivalent to
10+2 Higher Secondary Examination after a period of 12 years study, the last two years of study comprising of Physics, Chemistry and Biology after the introduction of the 10+2+3 years education structure as recommended by the National Committee on education;
 OR
2. The intermediate examination in science of an Indian University/Board of recognized examining body
with Physics, Chemistry and Biology which shall include a practical test in these subjects and also English
as a compulsory subject;
OR
3. The Pre-professional/pre-medical examination with Physics, Chemistry and Biology, after passing either
the higher secondary school examination, or the pre-university or an equivalent examination. The pre professional/pre-medical examination shall include a practical test in Physics, Chemistry and Biology
and also English as a Compulsory Subject.
 OR
4. The first year of the three-year degree course of a recognized university with Physics, Chemistry and
Biology including a practical test in these subjects provided the examination is a ‘University Examination’
and candidate has passed 10+2 with English at a level not less than a core course.
 OR
5. *B.Sc. examination of an Indian University, provided that he/she has passed the B.Sc. examination with
not less than two of the following subjects-Physics, Chemistry and Biology (Botany, Zoology) and further
that he/she has passed the earlier qualifying examination with the following subjects-Physics, Chemistry
and Biology and English.
 OR
6. *Any other examination which, in scope and standard is found to be equivalent to the intermediate
science examination of an Indian University/Board, taking Physics, Chemistry and Biology including a
practical test in each of these subjects and English.
* Candidates will have to obtain eligibility from the Baba Farid University of Health Sciences, Faridkot, Punjab.

Note: 1 Punjab domicile candidates should have passed his/her 10+1 and 10+2 examination or other qualifying examination (as listed above) as regular candidate from a recognized institution situated in Punjab only. The candidate would be required to submit a certificate to this effect from the Principal/Head of the Institution last attended in the prescribed Performa.
The candidates who have studied as regular students both for 10+1 and 10+2, for two years in a recognized institution situated in Punjab and have appeared in the 10+2 examination from that institution but due to failure have cleared the 10+2 examination in supplementary batch, or subsequently are also eligible subject to the condition that they clear such examination before submitting form for Entrance Test or if they have appeared in the examination, at the time of submissions of the form for entrance test and their result of 10+2 is yet awaited, they submit a proof of having cleared the said examination at the declaration of the awaited result with requisite eligibility conditions.
Note: 2 In case the candidate has also studied mathematics along with Physics, Chemistry, Biology
and English then the marks obtained in Mathematics are not to be considered for the purpose of calculation of 10+2 marks for consideration of merit.
Note: 3 Examination of universities / board / body outside India
Applicants seeking admission on the basis of qualification obtained in foreign countries from a University/ Board outside India shall apply to the Registrar Baba Farid University of Health Sciences, Kotkapura Road, Faridkot-151 203, Punjab., India http://www.bfuhs.ac.in  to receive an Eligibility certificate for applying for admission to 1st year Undergraduate courses of this college. If a true copy of the eligibility certificate does not accompany the detailed marks of the qualifying examination for admission, the application will not be considered. However, all applicants who are hopeful of getting eligibility certificate from Baba Farid University of Health Sciences can appear in the entrance test provided the fee for the test has been paid by the applicant.

HOW TO SUBMIT APPLICATION
Application process for UG AWT 2014 is Online only.
On-Line Application: Applications are to be submitted on-line by accessing the website:
www.cmcludhiana.in  from 1-04-2014 to 14-05-2014. The Prospectus can be downloaded from the available link. Every candidate will be required to log on to ‘Admissions 2014’ on the institutional website.
The online application form must be filled and submitted online (mandatory). This must be followed by submission of signed print out of application form (mandatory) along with required enclosures.
(Online application form will be available till 14th May 2014).
Instructions for On-line Applicants
Log on to www.cmcludhiana.in and navigate to link “UG 2014 - MBBS/BDS/B.Sc Nursing/BPT” and follow the instructions given.
After successful registration, your application number will be assigned. Note this down for any future reference.
i. After filling the Online form, upload your recent photograph in JPG format only (preferably of aminimum size of 295 x 356 pixels) in the appropriate space provided and submit. The photograph should have a plain background and should not be altered by photoshop or any other software.
ii. Take the print-out of your online Application form on an A4 size sheet.
iii. Staple two recent passport size photographs in the appropriate spaces on the printed form and sign
in the space provided for ‘signature of the candidate’.
iv. Attatch the Demand draft to the form.
v. Completed Application with enclosures should be sent to The Registrar, Christian Medical College Ludhiana-141008, Punjab, so as to reach on or before the prescribed closing date, 16-05-2014.
vi. Candidates applying in the Christians with service agreement category in MBBS need to download and fill the service agreement proforma avalaible online and send to the Body/Church concerned.
Applications received afer the due date for any reason including postal delay, shall be rejected.
The institution will not be responsible for any delay or loss in transit by postal department, courier agencies etc and proof of posting will not be considered as proof of receipt of the same.
Note: Applications must be sent in the prescribed format for this admission. Applications sent in any other format (Xerox copies) shall be rejected. Failure to sign the application form and enclose the Demand Draft will lead to rejection of Application without any further correspondence.
Admit cards will be processed only after receipt of the above at the office of the Registrar, CMC Ludhiana by the last date for submission.
Acknowledgement of online submission alone does not indicate acceptance of application and has no bearing on the admission. Acceptance follows receipt of Application form, enclosures and demand draft (by post/courier/hand delivery) in the office of the Registrar before the last date of submission and subsequent verification. In the event the application form with enclosures and demand draft does not reach the office of the Registrar before 16th May 2014, the application stands cancelled.
Receipt of Admit Card does not mean that the applicant has met all the eligibility criteria for admission but indicates only eligibility to appear in the examination

1. The period of training for all the above mentioned courses is 3 years and will be as per rules of the Medical Council of India and Baba Farid University of Health Sciences, Faridkot to which the Christian Medical College is affiliated.
 2. Academic requirements:
 2.1. For M.Ch courses an applicant must have obtained a postgraduate degree in Surgery (MS) from a recognized University / Body as per the requirement of the Medical Council of India and Baba Farid University of Health Sciences, Faridkot, Punjab.
 2.2. For DM course an applicant must have obtained a postgraduate degree in Medicine or Pediatrics (MD) from a recognized University / Body as per the requirement of the Medical Council of India and Baba Farid University of Health Sciences, Faridkot, Punjab.

 3. General :
 3.1. All the seats are open for all courses
 3.2. Candidates must be free from any service obligation and must produce a service obligation completion and no-objection certificate from the appropriate authority/sponsoring body if sponsored for under graduate (MBBS) and/or post graduate (MD / MS) course.
 3.3. Candidates must produce a no-objection certificate from the appointing authority where working, if applicable.
 4. Registration
 4.1. Must be registered with the State Medical Council / Medical Council of India.
 5. Nationality
 5.1. Must be a citizen of India.
 6. Service obligation and No Objection Certificate (NOC):
 6.1. An applicant of CMC Ludhiana who is expected to complete the service obligation (if any) in lieu of Sponsorship (for PG) by 30-06-2014, may apply. A letter to this effect must be provided along with the application from the Director of Christian Medical College, Ludhiana.

6.2. Mission sponsored MBBS graduates of Christian Medical College, Ludhiana must produce a letter to this effect from the authorized representative of the sponsoring Body, mentioning the period of service rendered by the applicant with dates and areas of service along with a no-objection certificate.
 6.3. For MBBS graduates and MS/MD postgraduates of other colleges, a no-objection certificate from the Principal or appropriate authority along with a certificate for completion of any agreement or obligation, if applicable, should be provided along with the application.
 6.4. Uncompleted Service Obligation: Candidates who have not completed their service obligation for any reason and those who have been released from the service obligation prematurely will not be eligible to apply for post graduate studies in CMC Ludhiana. The primary aim of the Christian Medical College Ludhiana is to make a significant contribution to the health standards of all communities of our nation with a special emphasis on health care in the rural underserved and unreached areas/communities in India. This mandate is fulfilled by graduates and post graduates who fulfill their sponsorship obligation by serving in such areas, participating in preventive and curative medicine and understanding the needs of the society. Non fulfillment of service obligation for any reason violates the spirit of sponsorship.

APPLICATION PROCESS
Prospectus along with application form and relevant papers will be available from 7th May 2014.
Prospectus and application form may be downloaded from the website www.cmcludhiana.in.  All the seats in the DM/M.Ch courses are open and available to all from India.

How to submit Application:
1. A complete application for postgraduate program contains:
a. Application form completely filled in with a photograph attached and signed by the applicant.
b. Two copies of passport size photographs self-attested at the back and attached to the form.
2. Application fee - A bank demand draft drawn in favor of “Christian Medical College Ludhiana Society” payable at Ludhiana should be sent along with the application form. (Demand draft amount is for Rs. Three Thousand Five Hundred only (Rs.3,500/-) Application fee once paid is not refundable.

3. Self-attested Photocopies of the following:
a. Internship completion certificate.
b. MBBS Degree certificate
c. MBBS Marks certificates - all University Professional examinations.
d. MD / MS Degree certificate
e. Registration Certificate with State Medical Council /Medical Council of India.
f. Matriculation or equivalent certificate for proof of date of birth.
g. Conduct certificate from the Principal of the Medical College last attended.

h. No objection certificate from the appointing authority where working, if applicable.
i. Service Obligation completion Certificate and No objection Certificate from the Sponsoring Church/Body/Institution, if applicable.
j. For Applicants working in Christian Medical College Ludhiana, self-attested Photocopy of certificate from the Director for satisfactory completion of service in College / Hospital as per Appointment along with a No objection certificate to apply (to be considered for calculation of additional marks, if applicable. 4. Photographs:
Each applicant must provide 3 copies of his/her recent passport size photograph showing face, head and shoulders.
1. One photograph to be pasted on application form.
2. Two photographs-should have the applicant’s signature and name written clearly in English on the back of each photograph and to be stapled in the area provided.
3. If selected for admission, the applicant is required to bring six stamp size color photographs, which is to be submitted at the time of scrutiny of papers & submission of fees for admission.
If the Supervisor/Invigilator at the examination center has any difficulty in recognizing the applicant from his/her recent passport size due to change in hair or beard style of the applicant (since the photograph was taken) the applicant will not be allowed to appear for the admission written test.


SUBMISSION OF APPLICATION
The application form(s) along with enclosures should (preferably) be sent by speed post/Registered post/by hand in the envelope provided, duly sealed, so as to reach the Registrar, Christian Medical College, Ludhiana-141 008, Punjab on or before 03.06.2014 (before 5.00 pm.). This may also be submitted, duly sealed, to the office of the Registrar by hand. In the latter case, a receipt must be obtained from the Office of the Registrar for the same.
No application will be accepted after 5.00 p.m. on 03.06.2014. Applications received after the due date for any reason including postal delay, shall be rejected. The institution will not be responsible for any delay or loss in transit by postal department, courier agencies etc and proof of posting will not be considered as proof of receipt of the same.
All applicants are required to complete the application forms themselves. Any mistake in filling the application form will lead to disqualification of the candidature.

Admit card:
The applicants should collect Admit Card for the Admission Written Test from the office of the Registrar personally, as per following schedule:

(1) The candidate must be an Indian National

(2) A candidate who holds MBBS degree from a medical college recognized by MCI or a foreign medical degree included in the schedules to the Indian Medical Council Act, 1956 and who has obtained full registration from the Medical Council of India or any of the State Medical Councils after completing compulsory rotating internship shall be eligible for admission to Christian Medical College, Ludhiana.

(3) Candidates who have obtained their MBBS from colleges which are not recognized by Medical Council of India will not be granted admission.

(4) An Indian National who qualified from a Foreign University/School must produce an eligibility certificate recognizing the qualifying examination as equivalent to MBBS as recognized by the Baba Farid University of Health Sciences and the Medical Council of India.This may be obtained from the

(5) A candidate who after passing the final qualifying examination (MBBS or the foreign Medical degree recognized by MCI) is undergoing 12 month compulsory internship / practical training and is likely to complete the same by 31.03.2012 can take the examination but he/ she shall not be eligible for admission unless he / she has completed the compulsory rotating internship and obtained full registration on or before 31.03.2012 or the candidate produces a certificate from the head of institution that he / she will be completing internship by 31.03.2012.

CMC LUDHIANA ENTRANCE ON MAY 27, 2011 (UG–AWT–2011)

A common Admission Written Test (UG–AWT–2011) will be held for the UG courses including MBBS by Christian Medical College, Ludhiana on 27th May 2011, from 2:00 pm to 4:30 pm at Allahabad, Amritsar, Bathinda, Calcutta, Chandigarh, Chennai, Kochi, Ludhiana , Mumbai, Nagpur and New Delhi . Bible Test for Christian applicants will be held from 4:45 pm to 5:15 pm, on the same day. Therefore, all Christian applicants applying in the sponsored category need to remain seated after the UG–AWT– 2011 till 5:20 pm.

Admission to MBBS, BDS, B.Sc. Nursing and BPT (collectively called as UG courses) of this institution, are on the basis of merit obtained in the Admission Written Test (UG – AWT - 2011) conducted by Christian Medical College Ludhiana Society.

Open Category

Any candidate who is an Indian National, irrespective of caste, religion or domicile, can apply under this category. They will not have a service agreement but are encouraged to serve in the rural mission hospitals to understand the basic needs of the country.

Christian (Minority Category) Applicants & Service Agreement Candidates who desire to apply under the Christian sponsored category should indicate in the ICR form and will have to take a Bible Test. Such candidates are required to be sponsored by one of the sponsoring agencies of the Christian Medical College Ludhiana society for eligibility in the sponsored category. Candidates can apply to one of the sponsoring agencies of the Christian Medical College Ludhiana Society for sponsorship.

List of members is given. Toward this, all MBBS, BDS, and B.Sc. Nursing candidates selected through UG AWT 2011 in this category, are required to enter into a service agreement without which admission will not be granted. Service Agreement for

MBBS and B.Sc. Nursing candidates is for a minimum period of two years and for BDS candidates, 1 year, after completion of the course in a hospital or institution managed by Christian Medical College Ludhiana Society or its member Body/Church.

Admission will be entirely on merit, based on the Admission Written Test.

Candidates must provide the certificates mentioned below to be considered for admission in Sponsored Christian (Minority) category. The following original certificates must be available at the time of scrutiny of certificates otherwise the candidature will be cancelled.
1) Sponsorship Letter from sponsoring body/church signed by authorized signatory only
2) Baptism certificate
3) Letter from the Pastor regarding church membership with dates (made after 28.02.2011 in the prescribed format See Annexure -I Sample-12)
4) Domicile certificate issued and certified by the competent authority
1. Christian candidates desirous of sponsorship should only apply to one sponsoring
body/church. In the event that a candidate has obtained more than one sponsorship, the first one to reach the office of Registrar will be considered.
2. It is mandatory for all Christian applicants to pass the Bible Test for considering them
under the Christian Minority categories. Christian applicants who fail in the Bible Test will not be considered under the Christian Minority category.

Eligibility criteria for UG AWT 2011

Age: Applicants must be at least 17 years of age on 31st December 2011 (the date of birth being shown on the Matriculation or Higher Secondary Certificate or equivalent certificate). Applicants born after 31st December 1994 are not eligible to apply for admission to Undergraduate courses starting in July 2011.

Nationality : Applicants must be citizens of India for UG AWT 2011. Persons of Indian Origin with Citizenship of other countries may apply under NRI category for which a separate prospectus is available. No other foreign national will be admitted.

Academic requirements : A candidate for admission to MBBS and BDS courses must have obtained not less than 50% marks in English and 50% marks in Physics, Chemistry and Biology taken together at the qualifying examination (as per MCI Regulations 1997).

For admission to B.Sc. Nursing and BPT course a Candidate must have passed in the subjects of Physics, Chemistry, Biology and English individually and must have obtained a minimum of 45% marks taken together in Physics, Chemistry, Biology and English in 10+2 examination. In respect of candidates belonging to Scheduled castes, scheduled tribes, backward classes the marks obtained in Physics, Chemistry, Biology and English taken together in qualifying examination be 40%.

General school information
Candidate must have passed qualifying examination as under: -
1.The higher secondary examination or the Indian School Certificate Examination which is equivalent to 10+2 Higher Secondary Examination after a period of 12 years study, the last two years of study comprising of Physics, Chemistry and Biology after the introduction of the 10+2+3 years education structure as recommended by the National Committee on education; OR
2.The intermediate examination in science of an Indian University/Board of recognized examining body with Physics, Chemistry and Biology which shall include a practical test in these subjects and also English as a compulsory subject;
OR
3. The Pre-professional/pre-medical examination with Physics, Chemistry and Biology, after passing either the higher secondary school examination, or the pre-university or an equivalent examination. The pre-professional/pre-medical examination shall include a practical test in Physics, Chemistry and Biology and also English as a Compulsory Subject.
OR
4. The first year of the three-year degree course of a recognized university with Physics, Chemistry and Biology including a practical test in these subjects provided the examination is a University Examination. and candidate has passed 10+2 with English at a level not less than a core course.
OR
5. *B.Sc. examination of an Indian University, provided that he/she has passed the B.Sc. examination with not less than two of the following subjects-Physics, Chemistry and Biology (Botany, Zoology) and further that he/she has passed the earlier qualifying examination with the following subjects-Physics, Chemistry and Biology and English.
OR
6. *Any other examination which, in scope and standard is found to be equivalent to the intermediate science examination of an Indian University/Board, taking Physics, Chemistry and Biology including a practical test in each of these subjects and English.

* Candidates will have to obtain eligibility from the Baba Farid University of Health Sciences, Faridkot, Punjab.

Note 1: Punjab domicile candidates should have passed his/her 10+1 and 10+2 examination or other qualifying examination (as listed above) as regular candidate from a recognized institution situated in Punjab only. The candidate would be required to submit a certificate to this effect from the Principal/Head of the Institution last attended in the prescribed Performa.

The candidates who have studied as regular students both for 10+1 and 10+2, for two years in a recognized institution situated in Punjab and have appeared in the 10+2 examination from that institution but due to failure have cleared the 10+2 examination in supplementary batch, or subsequently are also eligible subject to the condition that they clear such examination before submitting form for Entrance Test or if they have appeared in the examination, at the time of submissions of the form for entrance test and their result of 10+2 is yet awaited, they submit a proof of having cleared the said examination at the declaration of the awaited result with requisite eligibility conditions.

Note 2 : In case the candidate has also studied Mathematics along with Physics, Chemistry, Biology and English then the marks obtained in Mathematics are not to be considered for the purpose of calculation of 10+2 marks for consideration of merit.
Note 3 : Examination of universities / board / body outside India

Applicants seeking admission on the basis of qualification obtained in foreign countries from a University/Board outside India shall apply to the Registrar Baba Farid University of Health Sciences, Kotkapura Road, Faridkot-151 203, Punjab., India http://www.bfuhs.ac.in to receive an Eligibility certificate for applying for admission to 1st year Undergraduate courses of this college. If a true copy of the eligibility certificate does not accompany the detailed marks of the qualifying examination for admission, the application will not be considered. However, all applicants who are hopeful of getting eligibility certificate from Baba Farid University of Health Sciences can appear in the entrance test provided the fee for the test has been paid by the applicant.

Candidates having qualifying examinations from schools / institution outside India are to produce proof of Indian citizenship (Passport only) at time of admission. It is upon the candidate to prove and support with appropriate documentation and certificates that he / she is eligible to apply under a certain category and for the courses mentioned. No documentation is to be produced for applying to appear in the UG AWT 2011 and as such information provided in the ICR form will be considered as true. BUT at the time of appearing for counseling the candidate will have to prove the facts given in the ICR form regarding eligibility to be admitted to a particular course in a particular category E.g. A candidate applying in category 8 for B.Sc. Nursing will have to prove that she is from Punjab as well as from Backward caste.
