UG / PG Pharmacy Admissions 2012-2013

JSS University, Sri Shivarathreeshwara Nagara, Mysore has invited applications for admission to UG & PG Pharmacy Courses (M.Pharm, B.Pharm, Pharm D. and Pharm D. (Post Baccalaureate) of Academic Year 2012-13.

Requisite Qualification for admission to M.Pharm Course :
Candidates who have passed B.Pharm degree examination of any recognized University and have secured not less than 50% of the maximum marks (aggregate of four years) prescribed for the qualification examination are eligible for admission to the M.Pharm course.

Requisite Qualification for admission to B.Pharm course :
• Pass in HSC/PDC/PUC (10+2) with an aggregate of 50% of marks in Physics, Chemistry, Biology (Botany, Zoology) or Mathematics and also English passed separately.

Requisite Qualification for admission to Pharm D. course :
• Pass in HSC/PDC/PUC (10+2) with Physics, Chemistry, Botany, Zoology or Biology or Mathematics (OR)
• Pass in D.Pharm examination from PCI approved institution.

Requisite qualification for Pharm D. (Post Baccalaureate) course :
• Pass in B.Pharm from any recognized University and approved by Pharmacy Council of India, New Delhi.

Application Procedure :
A. PG-Pharma :
• Candidates are required to complete the prescribed application form and submit the same to the University on or before the scheduled date. The downloaded and printed application form should be sent along with a processing fee of Rs. 2,000/- by way of DD favouring "JSS University Fee Account, Mysore," payable at Mysore along with the relevant documents.
• Candidates may also obtain the application form along with Brochure by sending a processing fee of Rs.200/- by way of DD favouring "JSS University Fee Account, Mysore", payable at Mysore. The duly filled application form should be sent to the University along with relevant documents and a processing fee of Rs. 2,000/- by way of DD favouring "JSS University Fee Account, Mysore", payable at Mysore.

• Printed brochure and application forms are also available at:
  JSS College of Pharmacy, Mysore – 570015, Karnataka.
  JSS college of Pharmacy , Ootacamund -643001, Tamil Nadu.
(Application forms can also be downloaded from : www.jssuni.edu.in ).

Attested copies of the following documents have to be enclosed along with the application form and must arrange the documents in the following order:
1. SSLC / 10th / Equivalent examination marks card.
1. Marks card (B.Pharm) from First to Final year.
2. Provisional / Permanent Registration of the Pharmacy Council India.
3. Degree Certificate.
4. PCI – recognition of the college from where student has completed the course.
5. Conduct / Character and Transfer certificate issued by the college last studied.
6. Date of Birth certificate if not mentioned in the SSLC/Equivalent examination marks card.
7. Caste / Community certificate.
8. Migration Certificate issued by the University.
9. Six recent, identical passport-size colour photographs with name and date. (In addition one copy of the photo should be pasted on the application form in the space provided).
10. Three self-addressed A4 size envelopes.
11. Foreign Nationals seeking admission should obtain eligibility certificate issued by JSS University, Mysore.
12. Processing fee of Rs. 2,000/- by way of DD favouring JSS UNIVERSITY FEE ACCOUNT, MYSORE, payable at Mysore. Candidates should write their name, address and "APPLICATION FOR ADMISSION TO M.PHARM COURSE" on the back side of the Demand Draft.

The completed application with requisite documents as detailed above along with demand draft for Rs.2000/-, should be sent to the under mentioned so as to reach before 5:00 pm on 15.06.2012, either by speed post, courier or by hand : The Registrar, JSS University, JSS Medical Institutions Campus, Sri Shivarathreeshwara Nagar, Mysore – 570 015, Karnataka.

B. UG Pharma :
Candidates desirous of admission to the B.Pharm, Pharm D. and Pharm D. (Post Baccalaureate) course is required to complete the prescribed application form and submit in either of the following ways:
• Candidates are required to complete the prescribed application form and submit the same to the University on or before the

• Degree certificate for admission to Pharm D (Post Baccalaureate).
• Provisional / Permanent Registration of the Pharmacy Council of India for admission to Pharm D (Post Baccalaureate).
• Conduct / Character certificate issued by the college last studied.
• Caste / Community certificate.

• Transfer certificate for Karnataka candidates.
• Migration certificate for other than Karnataka candidates.
• PCI – recognition of the college from where the tudent has completed the course for admission to Pharm-D (Post Baccalaureate).
• DD of Rs. 1,000/- favoring JSS University Fee Account, Mysore (For Application forms downloaded from the University Website).
