The Sanjay Gandhi Post Graduate Institute of
Medical Science (S.G.P.G.I.M.S), Lucknow invites application on prescribed
format for the following Diploma Courses at School of Telemedicine &
Biomedical Informatics, S.G.P.G.I.M.S 1. Telemedicine, 2. Hospital
Information Management System, 3. Public Health Informatics, 4. Digital Medical
Library, 5. Nursing Informatics. All the details regarding the courses are

Application Form correctly filled in on line on prescribed format and printed
copy along with demand draft of Rs. 500/- drawn in favour of Director,
SGPGIMS, (Academic Account) payable at State Bank of India SGPGI Branch, code
No. 7789 Lucknow and supporting document/certificates should be submitted
to the Executive Registrar, SGPGIMS, Lucknow by registered post/speed post/courier on or before 7th July 2013. Entrance test (MCQs pattern) is scheduled on 14* July 2013 at
10:00 AM in School of Telemedicine & Biomedical Informatics, SGPGIMS,
Lucknow. Candidates are required lo mention their e-mail address in the form
since intimation for the entrance test will be intimated through e-mail. No
separate call letters will be sent by post. The Director, SGPGIMS reserves the
right (o reject any or all applications without assigning any reason.
